Frequently Asked Risk Management Questions

A certificate of insurance provides evidence to the certificate holder (the party to whom the certificate is issued so it has confirmation that there is insurance if needed). The certificate of insurance shows the amount and type of insurance provided.

When there are contractual or statutory requirements that require evidence of insurance.

For example, a Central Student group is going to visit Stanley Black & Decker. Stanley may ask for a Certificate of Insurance or "COI" (evidence that Central has insurance) as part of approval process for the visit). 

Providing a Certificate of Insurance to another entity or organization that Central students, employees, etc. will visit is common. Another example: public schools or health providers typically require Central Students to provide evidence of insurance as part of the agreement between Central and other parties (schools/medical providers). The school or health organization will ask Central to provide a Certificate of Insurance so it knows that Central, through the State of Connecticut, carries insurance to satisfy various obligations, such as liability or loss or damage to property.

or also from https://www.ccsu.edu/fiscalaffairs/risk-management and completing the Certificate of Insurance Form and emailing it to Insurance@ccsu.edu. Central Risk Management will review the form and provide it to the SIRMB.

Central Departments should be prepared to provide the necessary information in writing to Central Risk Management via Insurance@ccsu.edu (and the SIRMB, if necessary), that will fully explain the need for the insurance and the intended purpose of the Certificate of Insurance.

No. It is not necessary for a state agency to get a Certificate of Insurance to use another state facility, since the State of Connecticut is protected by governmental immunity.

An additional insured is an entity other than the named insured (in our case, the State of Connecticut) who is protected under the terms of the insurance policy. Additional insured are identified by name in the description section of the Certificate of Insurance.

For example, the Central hockey club wants a non-Central ice rink to play a game. The ice rink will likely ask Central to name the ice rink as an "additional insured" so that the facility will have access to Central's insurance if something happens. 

A common Risk Management Form required to be completed and signed by Central students, faculty and staff, and non-Central participants participating in / attending Central sponsored programs, trips, events, and activities. (Note: formerly the Central Waiver Form.)

For example, a Central professor is taking Central students to visit a museum in Hartford. The professor and each student (or their parent or guardian if under 18 years of age) would fill out and sign the Assumption of Risk & Release Agreement. 

Central Assumption of Risk and Release

A Risk Management form required to be completed and signed by any non-Central employee over 21 whois attending Central sponsored programs, trips, events, and activities in the capacity of a Travel Chaperone.

Waiver - Chaperone

  1. Assumption of Risk & Release Agreements are needed for each person. If chaperones are involved, the Chaperone Agreement is required. Below are the standard processes to be followed:
  2. Department will fill the fillable fields on the Assumption of Risk & Release and/or Chaperone Agreements with the event/activity information.
  3. Department will provide the forms to all Central Students, Staff, Chaperones, parent/guardian (if applicable), and non-Central participants to complete and sign the Assumption of Risk and Release Form.
  4. If Chaperones are involved, Department will ensure that all chaperones are over 21 and complete and sign the Chaperone Agreement.
  5. Once all required forms are completed and signed, Department will scan all forms into one PDF file and send the file to Insurance@ccsu.edu no later than five (5) business days before the event/activity.
  6. Department must maintain original form(s) with the department's files as required by the Records Retention policy.
  7. Risk Mgt will review the forms and securely store the PDF file in Secure Apps.

If the site to be visited requests a Certificate of Insurance from Central or requires that Central name it as an additional insured, follow instructions for in #3 above- 

Note that Certificate of Insurance requests require a minimum of 2 weeks notice.

Complete a Certificate of Insurance Request Form and email Insurance@ccsu.edu a minimum of two (2) weeks prior to the sports clinic start date.
