Travel Survey Form

All full-time faculty intending to travel for a conference must complete this form, regardless of funding needs. This information is required for two reasons:

• To ensure proper allocation of AAUP travel funds.
• To allow departments to plan for continuity of teaching and other responsibilities.

We would like you to have a good idea of your travel plans before filling out this form. Please complete the Travel Survey Form by October 10, 2025.

CHRS Request for Additional Funds Form

To request supplemental travel funding, complete and promptly submit the attached Request for Additional Funds Form. This must include the justification and estimated costs exceeding the standard AAUP maximum allocation of $1500.
