After reviewing the materials above, complete and return the following items to the Payroll Department in Marcus White Annex, Room 006.  If you expect your payroll status to change in the future, you may submit the following forms to the Payroll Department. 

• Form I-9, Employment Eligibility Verification
• Form W-4 (Federal)
• Form CT W-4
• Direct Deposit and ACH Payment Enrollment Form
• Student Help Program Certification 
• Wage Justification Form
