Medical Information Requirements for Incoming Students
Please note that a complete and signed Connecticut State University Student Health Services Form is mandatory and is your only acceptable proof of vaccination to meet these requirements. University Health Services will not accept copies of immunization records instead of a completed form. Dates should be entered where required, not the phrase, "see attached." Please make this clear to your healthcare provider's office when you drop off the form. We appreciate the time and effort this takes both for you and your healthcare provider's office.

FULL-TIME STUDENTS: All full time students are required to submit a completed Connecticut State University Student Health Services Form prior to matriculating. For most students, proof of adequate immunization against measles, mumps, rubella (MMR) and varicella (chicken pox) along with completion of the Tuberculosis (TB) Risk Assessment are required prior to being able to register for classes. Full instructions are included with the "Full-Time Students Form." The instructions include age related and other exemptions, so please read this carefully. The form should be downloaded from this link, "Full Time Students (PDF)" or found at www.ccsu.edu/health/forms. Please submit the completed health form as soon as possible to avoid any delay in registering for classes.
PART-TIME STUDENTS: All part-time, matriculated students are required to complete page one of the Connecticut State University Student Health Services Form. For most students, proof of adequate immunization against measles, mumps, rubella (MMR) and varicella (chicken pox) is required prior to being able to register for classes. Completion of the tuberculosis risk assessment at the bottom of the form is also required. Full instructions are included with the form and include age related and other exemptions, so please read this carefully. Also, your medical provider needs to sign this form. No pre-admission physical exam is required. Page two is optional for part-time students. This information will allow us to better care for you if you are ever seen in health services. VARSITY ATHLETES: In addition to the requirements for full-time students, varsity athletes must complete four other steps to be medically cleared to participate in our varsity athletics' program. MENINGITIS IMMUNIZATION REQUIREMENTS FOR RESIDENTIAL STUDENTS AND RECOMMENDATIONS FOR ALL OTHERS:
All students living in campus housing are required by state law to be immunized against meningitis. Students will not be allowed to move in to the on-campus residential halls without proof of adequate immunization against bacterial meningitis. Students must have received a vaccine that protects against the four subtypes A, C, Y, and W 135. University Health Services recommends immunization against bacterial meningitis for all students and that you receive a booster shot if you were originally vaccinated more than 5 years ago. It is also recommended students be fully immunized against Hepatitis B. Please speak to your healthcare provider about these immunizations.

All full time students must be covered under a health insurance plan. If you do not want the University-sponsored health insurance provided by Aetna, you must submit a waiver or you will be billed for the insurance. This must be done at the start of each new school year.
