Complete the NIH Ethics Tutorial and retain a copy of your completion certificate.

Finalize your Submission - the form and all supporting documents (recruitment and data collection materials, informed consent document(s) and, if applicable, gatekeeper letter(s).

Email your complete New Protocol Submission to HSC@ccsu.edu *

* Faculty Advisors and Student Investigators: As of September 2011, advisors are to forward student submissions directly to the HSC as documentation of their review and approval of the student project.

Continuing Review

• Changing a protocol previously approved by the HSC?  Complete a Protocol Modification Request Form. 

• Continuing your research beyond the HSC approval expiration date?  Complete an Approval Renewal Request Form.
