                  Requesting Accommodations

1)     Submit documentation of a disability to the Coordinator of Student Disability Services. Documentation must be from a Licensed or Certified Professional.

2)     Following priority course registration, (Mid-November for spring semester and mid-April for fall semester), complete the REQUEST TO NOTIFY INSTRUCTORS OF ELIGIBILITY FOR REASONABLE COURSE ACCOMMODATIONS and return it, with a copy of your course schedule attached, to the Student Disability Services Office.

3)     Student Disability Service Office evaluates the requested accommodations and generates letters to faculty that provides suggestions for accommodations.

4)     Pick up accommodation letters during the first week of class.

5)     Meet with each faculty to engage in an interactive process to determine appropriate and reasonable accommodations for each course.
