Classroom and Lab Budget/Request Forms

The request forms described below are to be used for classroom and lab budgeting and planning only. To access the request forms, navigate to our IT Service Request portal at  https://hsm.ccsu.edu and log in with your BlueNet account username and password. Click on the name of the appropriate form.

Classroom/Lab Request for Funding - Software Request Form:

Please use this form to request the purchase of software for classroom or lab use. Be sure to consult the List of  Installed Classroom Software (found on the ITC web page here) before submitting the request form. If you do not see a software application that you need, please request it. If you have specialized software that has been installed in the past, please do not assume it will still be there in Fall. Please request it again so that we are aware of your needs. This form should be used for new purchases, upgrades costs, and maintenance costs.  Please submit one form per software requested. The information  gathered is important in the planning process. IMPORTANT NOTE: New software funding requests will be considered if  resources allow. Upgrades and maintenance costs necessary for the
continuance of curriculum take precedent.

Software funding requests need to be submitted for consideration annually by December 1st to be considered for the following fiscal year which begins July 1.

How to Request Software Funding: Click on Classroom/Lab Request for Funding - Software and log in with your BlueNet account username and password if prompted. Complete the online form and click on Review & Submit; confirm the information and click on Submit.

Classroom/Lab Request for Funding - Hardware Request Form:

Please use this form to request the purchase of hardware or other technology for classrooms and/or lab use. This form should be used for new purchases only. Please submit one form for each request. The information  gathered will be reviewed and prioritized by a subcommittee of the ITC. The subcommittee will make recommendations to the CIO and Provost for budgeting considerations.

Hardware Technology requests are ongoing and will be reviewed and prioritized on a regular basis by a subcommittee of the ITC. Requests received after December 1st may have to wait an additional budget cycle.

Classroom and Lab Software Installation Request Form

Requests to have software you purchased or freeware installed in a classroom or lab are required to be submitted at least three weeks in advance. This timeframe gives IT a chance to thoroughly evaluate the software to assure it will work well in our environment.

How to Request Software Installation: Click on Software Installation Request for Classrooms and Labs and log in with your BlueNet account username and password if prompted. Complete the online form and click on Review & Submit; confirm the information and click on Submit.
