HIPAA Information

If you propose to collect individually identifiable health information (known as Protected Health Information or PHI) from a health care or other entity that is covered by the new federal health care privacy law  known as HIPAA (Health Insurance Portability and Accountability Act) you will need to submit your HIPAA-related information to the HSC along with your standard HSC information.  For further information see:
