Welcome Incoming Varsity Student Athletes

 

Welcome all Varsity Athletes.

In order for you to participate in our Varsity Athletic Program, in addition to our health form sent to 
you shortly after your acceptance, we require a preparticipation office visit.  This preparticipation visit 
is scheduled in our office no sooner than 6 weeks prior to the start of your preseason.  If you live farther 
than two hours away from the campus, we realize you may not be able to come here during the summer 
for this appointment.  The appointment is about one and one hours long.  In those cases when you are 
unable to come to our office due to the travel distance beyond a two hour radius, we will contact you 
by phone to obtain some information that is evaluated at the time of the preparticipation visit.  Should 
you prefer to drive the distance, we appreciate your willingness to do so.  We understand that this may 
not be possible for some of you.  We then will need to see you to finish gathering other information 
when you come to campus on the first day of preseason.

 We have posted our requirements for your assistance concerning:

  • Our Health Form Requirements

      This form is to be filed NO LATER THAN  the early summer (June- early July) of each fall academic year. It will not be accepted at the time of your preparticipation visit that is scheduled in our office during the summer months.

    • We ask that your physician submit any additional information regarding any (ie. an echocardiogram, EKG, blood pressure monitoring, abnormal heart beats) or orthopedic evaluations (i.e. surgery, physical therapy) along with your medical form.
    • The more information we have before you come to your preparticipation visit, the more easily we can evaluate you for clearance to participate on the Varsity team.

  • Pre-participation Clearance Visit – a 1.5 hr appointment – free  
    • This appointment is focused on medical areas of concern to ensure the safety of each athlete in their field of play.

    • It is not done by other than our office medical staff.

    • It is scheduled by the CCSU medical staff only after the health examination report is submitted and is complete in the University Health Service.  
       
       
    •  If we require any additional medical evaluations or information we feel necessary prior to the start of preseason, we will inform you at the time of your visit what this entails.  You will have the summer weeks to do so.   

All of the above must be taken care of before you will be eligible to be cleared medically for your sport.
