Health Information Requirements

We congratulate you on your willingness to experience another culture and college experience at our University.  We appreciate that it is not easy to live in another country.  We will try to do everything we can to help you maintain your health and mental well being while attending the University.

Listed below are key points to help ease you into life at the University:

  •       REQUIREMENTS PRIOR TO ATTENDING

  •       ORIENTATION

REQUIREMENTS PRIOR TO ATTENDING THE UNIVERSITY:
THE MEDICAL FORM:

International students who are full-time, part-time, or exchange students and those taking IELP courses at the University are required to submit a completed medical form before attending the university. This health form includes immunizations required for students. A Tuberculin Skin Test (Mantoux test) is also required and must be administered in
an American medical facility.

If you are planning on living in a the residence hall on campus, you will be required to have the meningococcal vaccination (Menactra).  This vaccination must also be administered in an American medical facility. The United States requires this vaccine with subtypes A, C, Y, W 135 in the vaccine, which may not be available in your country.

The University Health Service will be offering both the Tuberculin Skin Test and the meningococcal vaccination
for those of you who are not able to obtain either of them in an American medical facility prior to the first day of
classes.  Additional immunizations required but not submitted in your health form can be scheduled to be completed
in our office.  There are fees for the test and the vaccinations that will be charged to your university account.

Please complete the form and return it to the University Health Service no later than January 15, 2008.  Additional
medical information may be needed, and we will notify you by e-mail to enable you to obtain this medical information
while you are still in your country.

  • The information provided on the Connecticut State University Health Service form is strictly confidential and will be reviewed and used only by health care providers employed by and affiliated with Central Connecticut State University.  The information contained within the health form is not shared with any staff person within the CCSU Center for International Education, U.S. immigration officials nor any other person at the University without your written permission to release any part of the information on the form from the University Health Service.

ORIENTATION
Each semester, a day of orientation is held for all international students prior to the start of the semester.  We are represented at your orientation and will go over the following information to help ease you into the American health care system:

  • The services we provide

  • Emergency: what to do.

  • Community health resources available

  • Health insurance questions.

HEALTH INSURANCE CONSIDERATIONS

  • All Central Connecticut State University students are required to maintain a health insurance policy that meets or exceeds the coverage provided by the Chickering Group/Aetna Health Insurance policy offered by the University.  While you may waive this policy by providing proof of alternate health insurance coverage, please be aware that MANY healthcare providers (physician and medical facilities) in the local area surrounding the University may not accept your form of alternate health insurance coverage, leaving you personally responsible for your medical bills or refusing to see you (even if you can afford to pay at the time of the visit).  Therefore, the Center for International Education and University Health Services strongly encourage all international students to purchase the Chickering/Aetna policy offered through the University.

We hope to meet all your medical needs while you attending our University.
