Dear Incoming Student for 2007-2008:


We wish to extend congratulations from the University Health Service upon your acceptance to Central Connecticut State University.

All full time students are required to submit this form to University Health Service no later than July 15, 2007 for the Fall semester and December 15, 2007 for the Spring semester. This form includes a health history, immunization record and a physical examination that must be completed by your personal physician. For those students accepted after December 15, 2007, we have extended the deadline until the first day of class. 

Our health form is mailed to you within a week following your acceptance to the University.

Guidelines for these requirements are stapled to the front of the form. If the form is submitted with any missing information, we will return it to you requesting the necessary data.

Please note re: TRANSFER STUDENTS:
When you request your information to be released from the old university, the medical records are not transferred with your academic record.  You must release that information from the health service separately and forward it to our office.  

During the school year, our office welcomes all students for medical appointments regardless of your insurance plan. Our appointments are free. The medical form is required whether or not you choose to use our office for medical care. In addition, we see all patients whatever your insurance coverage is. We do not third party bill.

PLEASE NOTE: Though we do not administer the insurance policy, information about the AETNA Life Insurance "NEW" Student Health Insurance Policy has been mailed to you from the AETNA Chickering Group.  The Chickering representative will be available during registrations in the summer for additional questions.
