The University Health Service medical records are confidential. Medical records or
information will not be released to anyone- administrative faculty, professors, family members,
spouses, and prospective employers without the patient’s consent.
"Confidentiality is primary in a
patient-health care provider relationship. In this health center,
medical records are kept on all visits and this information is solely
for the purpose of caring for you. Information regarding your visits
here may not be released nor discussed with anyone, nor have information
requested from your record without your written consent or verbal
consent during your appointment. This means that any information
learned during your visit is never released to faculty, staff, friends,
or family members unless you authorized consent. Similarly, any medical
information to be forwarded to another medical facility or university
requires your written request."
Patients Rights and Responsibilities
RIGHTS OF PATIENTS
1. Patients are treated with respect,
consideration and dignity.
This approach to the patient is begun at
the first contact with the patient, whether it be for information, an
appointment or health inquiry by the patient.
2. Patients are provided with
appropriate privacy.
Patients are seen by a health care
provider in the health care providers office setting. If a patient
chooses to be with another health provider, it will be honored after the
initial contact has been made by the provider and the patient alone
wherever possible.
3. Patient disclosures and records are
treated confidentially, and, except when required by law, patients are
given the opportunity to approve or refuse their release.
There are signs throughout the health
area explaining to the patient the definition of confidentiality.
When records are requested by an outside
agent, we will obtain release of information from the named patient
before providing the information to the requested agent. This includes
requests for immunizations or any part of the medical record.
When records are subpoenaed, our office
will notify the Office of Student Affairs that a subpoena was issued for
medical records for a particular individual. We will comply with the
subpoena request.
4. Patients are provided, to the degree
known, complete information concerning their diagnosis, evaluation,
treatment and prognosis.
When it is medically inadvisable to give such information to a patient,
the information is provided to a person designated by the patient or to
a legally authorized person.
All patients seen will be given
information and a plan for treatment for their medical diagnoses. There
are handouts available for several diagnoses that supplement medication
and/or oral instructions given at the time of the appointment. If lab
tests are ordered, the patient will be contacted within 24 hours of our
office receiving the results. This will be explained to the patient at
the time of the appointment.
5. Patients are given the opportunity to
participate in decisions involving their health care, except when such
participation is contraindicated for medical reasons.
If a patient is deemed incompetent to
make decisions for him/herself, the Medical staff caring for him/her
will document this in the chart and make the necessary medical
arrangements to send the person to the appropriate medical facility or
caretaker.
6. Information
available to the patients as to patient rights, services available,
provisions for after-hours and emergency care, fees for service and
payment policies.
We will provide a
list of our services available in the main waiting room of our office.
We also will provide information for alternative medical facilities when
we are not open. For those facilities to whom we refer, we will call
ahead to the medical office whenever possible to help facilitate an
appointment.
