The following forms can be downloaded and submitted to Human Resources in Davidson Hall, Room 119:

NOTICE: You are submitting this Form W-4 under the penalties of perjury. This means that if you knowingly claim allowances or an exempt status to which you are not entitled, you are subject to criminal prosecution in Federal Court. Submission of a fraudulent Form W-4 is a federal crime under Title 26, U.S. Code Section 7205, that is punishable by up to one year in prison and a fine of up to $100,000. In addition, knowingly submitting a false Form W-4 may constitute evidence of tax evasion (or other federal crimes). Tax evasion is a federal crime under Title 26, U.S. Code Section 7201, that is punishable by up to five years in prison, a fine of up to $1 Million, costs of prosecution, and an order of restitution for any federal taxes owed. 

If you claim an exempt status, the following form must also be completed:

Instructions for setting up Direct Deposit/ACH via the Web (click here for printable version of instructions):

  1. Navigate to the CCSU home page. Point to CentralPipeline.

  2. From the CentralPipeline home page, click on the WebCentral-Banner Web link and log in with your BlueNet account username and password.

  3. From the Home tab, click on the Direct Deposit Enrollment for Payroll/Travel/Stipends link.

  4. To enroll in Direct Deposit/ACH, enter the bank routing number and account number and click the Save button.

  5. To modify or cancel an existing direct deposit record, select the bank name from the list and follow on-screen instructions.
